The Northeast Florida Community Action Agency, Inc. (NFCAA) is taking steps to maintain the health and safety of our customers, our employees, and our communities.
NFCAA has temporarily suspended in-person operations to prevent the spread of COVID-19. Access to our offices is limited to authorized personnel until further notice.
As a result, we encourage you to use the self-service options provided below. All appointments and applications must be completed online or by mail for our service areas in Baker, Clay, Duval, Flagler, Nassau, Putnam and St. Johns County.

STEP 1: APPOINTMENT.
You are required to make an appointment for ALL services before completing step 2. Applications completed without an appointment will not be accepted for processing. Appointments can only be made for the county where your residence is located. You can make an appointment through the Online Appointment Scheduler or call 904-362-8052. The appointment line is available 24 hours, 7 days a week. Appointments can be scheduled up to 14 calendar days in advance. A live operator will be available for customers who require assistance between the hours of 10 a.m. – 5:00 p.m. Eastern Standard Time. Language accommodations will also be available. A NFCAA representative will contact each individual on the designated appointment day and time to conduct the intake process over the phone. This process addresses the following:
- ASSIST INDIVIDUALS THAT ARE UNABLE TO FILL OUT THE ONLINE APPLICATION
- SETUP A DAY AND TIME FOR INDIVIDUALS TO DROP OFF/SCAN REQUIRED DOCUMENTS IN THE DESIGNATED OFFICE
- PROVIDE ANY ADDITIONAL ASSISTANCE TO THE INDIVIDUALS
- ELIGIBILITY OF SERVICES/PROGRAMS
STEP 2: APPLICATION.
Using the Online FASTRACK System is the quickest way to expedite your services (all programs). You must upload the Required Documents into the system by scanning or taking pictures of the documents. All household members age 18 and over with no income must complete the Self-Declaration Form. Social security numbers of applicants and household members are requested using the Notice Regarding Collection of Social Security Numbers Form. This information is necessary to determine eligibility for program services and specifically for the following purposes: 1. To verify an applicant’s identity. 2. To verify household size. and 3. To verify household income.
For Utility Services Only
Please complete the Utility Bill Certification Form if requesting utility services and the bill is not in the customer or household member’s name. The person whose name is on the bill must complete and sign this form. Customers that live in Clay County must complete the City of Green Cove Springs Utility Form.
